What is the fee and what is required..can you get it on arrival or must you
get it before you leave the US?

I suspect it depends if you're staying more than 30 or 60 days but here is
Vietnamese Embassy reply to me only last week (we are staying over 30 days
but not 60):

"Fee for a single entry 03 month visa is $110
Fee for a multiple entry 03 month visa is $150"

When I was asking the same question, their link to the information about the
visa was broken on their site.  I have no idea about getting one when you
arrive.  In our case, we are coming in around 10:30 pm, so there is no way I
am going to chance being held up all nght at Nobai airport.
