Do I need a special "pass" (vignette) to drive on some roads in France
and Spain? if so, where can I get one.  I'll be arriving in Madrid and
renting a car there and returning it there after three weeks but will
be driving to France.

In which cities is a rail pass good for travel via S-Bahn?  I seem
to remember reading that this is only true in Munich and Berlin,
but what about Frankfurt?

I've been reading about driving through France to Switzerland
and on to Italy.  Seems there are tolls in France and Italy which
is easy enough because they're both Euro currency.
What I'm a little unsure of is:

My wife went to www.raileurope.com, a US-based site, and found a train
Budapest to Prague, 23/10/2007, 09:50, single
but couldn't book it because UK didn't appear in the Countries list.

Some of our friends are taking a cruise in September in Europe.  At the end
of the cruise, they are planning on staying for a few days at the Hotel
Silberhorn in Lauterbrunnen, Switzerland.  Has anyone had any experiences
